All our information about this plaque, including the photograph, comes from (2016) . Another photo there shows the plaque attached to one of the gate piers that used to stand in front of the church but were removed 2019-20, along with the railings, to provide the ramped access.

From Google Street View one can see that the plaque was erected 2015 between April and Sept, and lost 2016 between May and October.
