Southall rail crash

An InterCity 125 high speed passenger train from Swansea failed to slow down in response to warning signals and collided with a freight train crossing its path. Seven people were killed and 139 injured. Great Western Trains (whose managing director survived the crash) were fined for failure to ensure their high speed trains had their automatic warning system working during long journeys. Procedures were changed following a public enquiry.
